Schedule Options: 9:00 AM – 9:00 PM | 9:00 PM – 9:00 AM | Flexible Days
Position Summary
Under the supervision of a Registered Nurse (RN), the Licensed Practical Nurse (LPN) provides health care monitoring and nursing interventions tailored to each individual’s unique needs. The goal is to support their ability to participate fully in daily programs and activities.
Key Responsibilities
- Monitor, observe, and document the health status of individuals served.
- Administer medications and perform nursing procedures as required.
- Conduct medical rounds and provide treatments as needed.
- Maintain accurate, timely, and organized documentation in all medical and agency records.
- Participate in interdisciplinary and health-related meetings.
- Support the RN in delivering health education to individuals, families, and staff.
- Attend continuing education seminars and required in-service trainings.
- Demonstrate compliance with all agency policies, procedures, and regulatory requirements.
- Learn and effectively use all software systems and technologies required by the agency.
- Maintain consistent attendance and punctuality; ensure responsibilities are covered during absences.
- Participate in team meetings and case reviews as assigned.
Supervisory Responsibilities
- Assist the RN in supervising staff in the delivery of medical care.
- May oversee staff administering medications.
Qualifications
- Current LPN license in the State of New York.
- Ability to travel to various program locations as needed.

Work Environment & Physical Demands
- Work settings include group homes, residential facilities, community settings, day programs, and clinics.
- Must be able to stand, sit, push, pull, lift/transfer individuals, and perform tasks requiring manual dexterity and visual acuity.
